Job Description:

The primary purpose of position is to in conjunction with the Social Service Director Plan, organize and develop the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations that govern our facility, and to ensure that the psycho-social needs of the residents are met/maintained on an individual basis.


  • Registered as a C.S.W. / Licensed Master Social Worker
  • Must possess the ability to work harmoniously with other department personnel, to ensure that a team effort is achieved in developing and providing a comprehensive plan of care. 
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents based on whatever maturity level in which that are currently functioning.
  • Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility. 
  • Promote resident/family participation in planning social service programs and discharge planning. 
  • Refer resident/families to appropriate social services agencies when the facility does not provide the services or meet the needs of the resident.
  • Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident. 
  • Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs. 
  • Review resident/family complainants and grievances from personnel, families or residents, and make written reports to the Administrator and Director of Social Services of action(s) taken within five working days. 
  • Review nurse’s notes/care plan summaries to determine if the care plan is being followed.  Participate in the review and revision of care plans and assessment as necessary, but not less than quarterly.  
  • Ensure that all personnel are aware of the resident’s right and responsibilities. 
  • Assure that appropriate notices are given to residents before their room or roommate is changed.
  • Provide residents with information concerning resident’s right to fair and equitable treatment, self determination, individuality, privacy, funds. Living wills property and civil rights, including the right to wage complaints, are well established and maintained at all times. 
  • Conduct discharge planning to effect safe community discharge.
  • Follow up as required by code with 30 days of discharge to verify discharge plan.
  • Perform other duties as assigned by Director or Administrator.

Shift Needs:

Day shift, must be able to work days, evenings and week ends as facility needs dictate.
