SOCIAL WORKER (SNF)

The Willows at Ramapo Center for Rehabilitation and Nursing is Rockland County's foremost sub acute care and long-term nursing center.  Situated on six lush, landscaped acres, The Willows at Ramapo Center for Rehabilitation and Nursing offers the finest in comfort, care and healing.  The Rehabilitation Pavilion is the largest sub acute community in the country, offering state of the art rehabilitation seven days a week in a unique spa -like setting.

We are in search of qualified candidates to fill an open Social Worker position we currently have available. 

Duties Include:

  • The Social Worker is responsible to provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being.

Requirements/Qualifications:

  • Bachelor’s degree in Social Work or similar professional qualifications with 2+ years experience; working directly with geriatric clients in a health care setting. Experience with MDS, CCP, and Discharge planning
  • Master’s degree in Social Work and/or Social Work license as regulated by NY state law with 1 year experience working directly with geriatric clients in a health care setting. Experience with MDS, CCP, and Discharge planning.
